Health Services at Thayer Academy

The Office of Health Services supports the mission of Thayer Academy by establishing policies and procedures to facilitate health and safety for the Thayer community, in compliance with Massachusetts regulations and in consultation with other Thayer health professionals and school administration.

The nurses...

  • Establish and maintain comprehensive school health records including required immunizations and copies of annual physical exam reports
  • Provide first-aid and illness assessment to students with acute illness.
  • Provide medically-prescribed interventions for students with chronic conditions, including medication administration.

Medical Forms

1. Medical Consent Form - Required Annually by July 1

Parents are asked to complete this online form every year by July 1. The Medical Consent Form can be completed here»

Instructions for completing the Medical Consent Form

  1. Your username [Provided in the June 12 email from the school nurse]
  2. Your current password [Also provided in the June 12 email from the school nurse]
  3. If you have difficulty logging into your account, please send an email to: nurse@thayer.org.
  4. Important Note: Each parent is sent a separate username and password. The parent who begins filling out the Medical Consent Form must be the one to complete and submit it.
  5. Once you log in, you will see your child's (children's) name(s) listed.
  6. Select Begin to the right of your child's name.
  7. Contact information on file at Thayer will automatically pre-fill the form, including, for returning students, information entered in last year’s form.
  8. Verify that the email address is correct. This is very important since Thayer uses email for most correspondence during the school year. 
  9. For each page, review the existing information, and revise/add as necessary.
  10. Select Save or Save & Next Page to proceed.
  11. At the end of the Student Medical Information page, scroll to the bottom and select Review to make sure the data is correct.
  12. You might be prompted to enter missing data.
  13. Scroll to the bottom and select Submit.
  14. You will then receive a confirmation email indicating successful completion.
  15. A Medical Consent Form must be completed for each child, so families with more than one child will need to fill out the form more than once.

Your health information is kept confidential, and is viewed only by those who are involved with your child’s medical needs at school. Exceptions to this rule depend on whether you child has...

  • A history of life-threatening allergies
  • Asthma
  • Or another serious medical condition 

If any of the above conditions apply to your child, information will be shared as needed with teachers, staff, and coaches to ensure your child’s safety.

2. Annual Physical Exam Report from your child’s physician

All students are required to have an annual physical exam submitted to the nurse in order to attend school.

  1. If your child has a physical exam report on file at Thayer, and it is dated within 12 months of September 2014, it is not necessary to send in a duplicate copy.
  2. If a physical exam appointment is scheduled during the summer, please indicate the date on the Medical Consent Form, and send in the updated physical exam report afterward.
  3. Physical exam reports need to be sent in as they come due during the course of the school year.
  4. A maximum of a 30-day extension is allowed past the annual date if needed for scheduling.
  5. Thayer's form can be used to submit your student's annual physical exam but most physicians provide their own form, and we accept forms from all doctors.

 

3. Epi-Pen Authorization Form

  1. If your child requires an Epi-Pen, please download and print the above required form, which must be completed by both the physician and a parent or guardian.

  2. Please provide at least 1 Epi-Pen (with a non-expired date), to be kept as a backup in the Health Office.

4. Inhaler Authorization Form

  1. If your child requires an inhaler, please download and print the above required form, which must be completed by both the physician and a parent or guardian.

5. Medications to be Dispensed at School - Authorization form

  1. This form must be completed if students need to take any medication while at school (other than EpiPens or inhalers, which require their own separate forms).
  2. The form must be completed by both the physician and a parent or guardian.

  3. Please contact one of the nurses to discuss the specific health needs for your child.
